AKC Updated Forms Therapy Titles/CGC
Attached are the current AKC forms required for those pursuing their therapy dog titles.
Please review and use the following:
• AKC Canine Partners Listing Application
(Required for mixed-breed dogs or dogs not currently registered with AKC)
• AKC Therapy Dog Title Application
(Required to apply for your AKC Therapy Dog title once visits are completed)
These are the updated forms directly from AKC, so if you are working toward your CGC and/or Therapy Dog title, these are the forms you will need.
Make sure you are using these versions moving forward.
